The NHS Type 2 Diabetes Path to Remission Programme is a 12-month, health coach–led programme designed to help you achieve and maintain weight loss safely, with the goal of putting Type 2 diabetes into remission. The programme is based on the latest research, including the DiRECT study, and is divided into three phases:

  1. Phase 1 – Low-Calorie Diet (12 weeks):
    You’ll replace your usual foods and calorific drinks with total diet replacement products, providing 800–900 calories a day. This allows you to lose weight safely and relatively quickly under supervision. Losing weight on this plan helps remove fat from your liver and pancreas, allowing them to work more efficiently, which in turn helps control your blood glucose levels and improves your overall health.
  2. Phase 2 – Gradually Restarting Food (6 weeks):
    You’ll slowly reintroduce ‘real food’ meals while continuing to follow healthy habits learned in Phase 1. This phase supports you to transition safely back to everyday eating while maintaining your progress.
  3. Phase 3 – Ongoing Support (8 months):
    You’ll continue to work with your health coach and receive resources to develop and maintain healthy lifestyle behaviours, helping you stabilise your blood glucose levels and maintain a lower body weight. This ongoing support is key to building on your achievements and putting diabetes into remission.

Throughout the programme, you’ll have access to one-to-one support from your health coach and high-quality resources, giving you the knowledge, skills, and encouragement you need to succeed during the programme and beyond.
